Summer Food Service Program (SFSP) Program Application

IMPORTANT DATES / DEADLINES
  • 3/17/25 – Application opens

  • 5/29/25 – Last day of training for sites to start by 6/9
  • 6/2/25 to 6/6/25 – Kitchen closed
  • 6/9/25 – 1st day of SFSP
  • 7/31/25 – All staff meeting; kitchen closed
  • 8/8/25 – Last day of SFSP

NOTE: Wherever the kitchen is marked as closed means that no meals will be delivered during that time. Sites may opt to pick up shelf-stable meals in advance if desired. Requests must be submitted to FFK@foodbankrockies.org at least two weeks in advance to ensure it can be accommodated.

If your site wants to be considered for the SFSP with Food Bank of the Rockies as your sponsor, please complete this application and return it with all required documentation.

  • The sponsor should complete this form for each site under its sponsorship each year. Sponsors of residential camps must collect this information for each camp session.
  • On May 17th, 2021, USDA released CACFP 11-2021, SFSP 07-2021 Collection of Race and Ethnicity Data by Visual Observation and Identification in the Child and Adult Care Food Program and Summer Food Service Program – Policy Recission. Racial and ethnic data may no longer be collected by visual observation in the SFSP.
  • Sponsors must collect self-reported data from participants to the maximum extent possible. Sponsors may use data that participants self-report through program enrollment, school data, census data, or CDE data.
  • All line items on this page must be answered numerically (no percentages).
  • The sponsor must retain racial and ethnic data for three years, plus the current year, and must safeguard this information to prevent its use for discriminatory purposes. Access to program records containing racial and ethnic data should be kept confidential and limited to authorized personnel.
